As a corporation that provides dental services, we adhere to a compliance program (a management system for the protection of personal information), handle personal information appropriately and conscientiously, and conduct highly transparent business activity. gPersonal informationh at the dental office includes questionnaires, consultation records, examination records, x-rays, intraoral photographs, denture molds, prescriptions, and referral forms. The group protects such personal information and makes efforts to provide safe and pleasant treatment. The group is also preparing to acquire a gprivacy markh for the protection of personal information.
The Tokushinkai Group upholds the following privacy policy in order to appropriately protect personal information
- The purpose of use of personal information is posted at the dental office.
- Personal information will not be handled outside of what is required to meet the purpose of use.
- When the purpose of use is revised, it shall be announced.
- The personal information shall not be acquired inappropriately.
- Personal information shall be kept accurate and up-to-date to the extent necessary for the purpose of use.
- Personal information shall be appropriately managed and protected from leakage, loss, manipulation, and unauthorized access.
- The consent of the patient shall be obtained upon providing personal information as part of a referral to third parties such as a private insurer, employer, or school.
- In order to provide adequate treatment to our patients, we may provide personal information to outside medical institutions and doctors to receive a second opinion. Inquiries are accepted by patients who wish to know about the matter.
- The group shall comply with the laws and regulations concerning the protection of personal information and conduct in-depth reviews in a timely manner in order to continuously improve our system concerning the protection of personal information.
- In principle, the group accepts requests for disclosure, amendments, and refusal of usage of collected personal information. Complaints and other inquiries concerning the handling of personal information are accepted at the "Personal Information Consultation Service"
- The group shall comply with the obligations set forth in the Personal Information Protection Act and other laws and regulations concerning the protection of personal information.
- The group shall continuously review the gPolicy Concerning the Protection of Personal Informationh and any related internal rules, endeavoring to improve the handling of personal information.

The Tokushinkai Group shall use personal information for the following purposes:
- The answering to questionnaires and the submittal of an insurance card is requested in order to prepare a consultation record and work journal for the dental hygienist.
- Personal information to be used for research, various examinations, diagnosis and presentation of treatment plan (explanation and consent), as well as lifestyle guidance.
- To be used for prevention, treatment, and oral functional recovery.
- To be used to adjust medical expenses.
- To be used to improve dental medical services by promoting maintenance and enhancement of health (e.g. mother's class, periodontal disease prevention class, etc.) and notifying patients of routine check-ups.
- To be used in cooperation with another hospital, doctor's office, pharmacy, and home care support centers.
- To be used to respond to another hospital, doctor's office, or pharmacyfs referral.
- To be used at a dental laboratory.
- To be used to explain medical conditions, treatment plans, post-treatment predictions, and lifestyle guidance, etc. to the patientfs family.
- Health insurance clerical work may be outsourced.
- Billing statements for insurance claims prepared by the doctor's office shall be submitted to a payment review facility in order to charge for medical expenses.
- Personal information to be used to respond to a payment review facility or insurer's inquiry.
- In the event that a company commissions a dental check-up, the results are submitted to the company.
- The group may consult or notify medical associations or insurance companies concerning issues relating to matters such as dental liability insurance.
- Personal information to be used as a basis to improve dental practice and business.
- To be used as part of a clinical case study at staff training workshop.
- To be used for the clinical training of doctors and dental hygienists.
